Who is eligible for services?
All children from birth to 18 years with complex needs and living in Tralee are eligible for services.
How can I get a referral to your services?
You can get a referral form from your public health nurse, GP and Health Centre. Referrals can be sent directly to the Tralee Team and are accepted from a variety of sources including
Parents/carers, Doctors, Allied health professionals ie speech and language therapists, School staff, Other children’s services ie Child and Adolescent Mental Health Services (CAMHS).
An application form and signed consent form along with appropriate reports are essential for the referral to be accepted.
