Person-centred planning is a process of goal setting with each service user in order to develop services based on their individual needs and wishes. It is based on five core values which are dignity, respect, choice, inclusion and support for the development of relationships.
The process creates a vision for a person life as part of their local community and plans for the actions needed to put this into practice. In addition the process looks to include all those people who are important to the individual so that they may assist and support them to reach personal goals and get a more responsive service.
